\n*Three passport photograph
\n*Copy of approved building plan, Copy of tenement rate receipt (occupied)
\n*Copy of letter addressed to executive secretary (LUAC), stating all documents attached as above. Typed with application address and collection of acknowledgementship (LUAC).
